Implant Supported Denture Process - Improvments

Within the last six months we have made several improvements in the materials and processes used for screw-down, fixed-detachable hybrid dentures and overdenture cases.

First, to ensure that we are processing at optimal levels we completely updated our premium Ivocap processing system: replaced flasks, injectors and the processing bath. Second, all bar cases are now processed on a separate analog model. Screwing the bar down at processing ensures the best result. Third, we redesigned our CAD/CAM bars and are now placing titanium posts within the long axis of all denture teeth for added tooth support. Fourth, the standard teeth we use in implant-borne removable prosthetics have been changed to Heraeus Kulzer, Mondial and Mondial i Denture Teeth. Research has shown that these teeth have a stronger bond to acrylic. In fact, these teeth are used by ClearChoice Centers and at the Malo Clinic, originators of the All-on-4 process. Finally, we now provide a vacuum-formed night-guard to protect the appliance from nocturnal parafunction. We believe these five improvements have added to the long-term success of our restorations and reduced the maintenance for doctor and patient.

All.In.One Fixed-Fee Implant Restorations

We are pleased to report that state-of-the-art CAD technology and our tech-savvy team of CAD/CAM technicians enable us to reduce fees and turnaround time on our premium, patient-specific, iRis custom abutment and bar implant prosthetics. We call our simple and straightforward fee structure All.In.One. It is intended to remove the uncertainty surrounding lab fees. These low fees, and our decades of restorative experience, can help you become more efficient and profitable in the restoration of implants while providing optimal treatment.

Fees for fixed restorations are per implant and include the following: special soft-tissue model, analog, articulation, iRis custom abutment, CAD abutment seating jig, and fixed restorations, including noble alloy. Price excludes high-noble alloy, shipping, and tax. Removable fees are per arch and include: custom trays, models, special soft-tissue model, articulation, standard and processed bite rim, fit-verification jig, setups, resets, cast metal or Fiber Force strengthening substructure, acrylic finish, one full set of premium teeth, one set of economy teeth, protective night guard, iRis titanium CAD/CAM bar on an unlimited number of implants, final screws for bar, analogs, and your choice of standard attachments (e.g., locators, hader clips, era, o-rings, bredent). Price excludes in-office technician labor, impression transfer, transmucosal and multiunit abutments (usually not required) shipping, and tax.

The fees represent maximum costs for the specified designs (call for new Implant Fee schedule). Actual fees may be lower depending upon processes that may not be required or performed clinically. Individual fees will be itemized and billed at the time service is rendered. All.In.One components are produced by ROE’s preferred suppliers. When original manufacturer parts are requested there will be an increased cost.

iRis abutments and bars featured in our All.In.One service are fabricated on the ULTRASONIC 20, a 5-axis, robotic milling machine capable of 42,000 rpm. The 2.3 ton machine is considered the most stable in its class and is capable of milling both titanium and zirconia from single unit to full roundhouse bridges, as well as implant bar frameworks, all within 10 microns.

Zimmer Angled Abutments - 30 degrees

Zimmer has announced release of their Angled Tapered Abutments. These abutments compensate for off-axis implants for expanded surgical options, such as All-On-4 type situations, to maximize the use of available bone, avoid the alveolar nerve and sinus, and minimize cantilevers for prostheses.

Separate Arches During CT Scan

It is imperative that the arches are separated during the CT scan. This goes for scan and no-scan appliance technique. Our radiographic guides include a radiolucent bite registration. However, if a bite reg is not present, use cotton rolls. Nearly all cases that do not have separated arches must be rescanned.

No Scan Appliance Technique - skip the wax-up

In the last few months we have completed hundreds of cases using our No-Scan-Appliance technique. This technique allows you to simply capture a CBCT and send models to our facility. From this we create surgical guides, saving you time, lab fees and appointments. When the models arrive at ROE they are digititized, and rather than completing diagnostic wax-ups, we design virtual teeth using crown and bridge virtual software. Once the teeth are set in ideal positions, the implant position is planned and we host our on line meeting to finalize the case.

Be sure to follow our protocol for case selection. Contra-indications for the no-scan-technique are edentulous patients and metal based restorations in the areas of proposed implant sites.

Happy New Year. We are excited to announce the availability of our NEW No Scan Appliance technique for planning your CT based implant guides. During the last several months we have worked hard with BlueSkyBio developing this appealing service for our clients to help save time and expense. We know you will be very pleased with this simplified service.

The process is quite simple. Capture a CBCT of your patient with the arches separated with cotton rolls. Be sure to remove any metal-based removable appliances prior to scan. Upload the DICOM images to www.dentalimplantplanning.com. Send master casts of both arches, a bite registration, and study model (if available) to ROE.

We will complete a diagnostic work up on your model to idealize tooth location. The work-up will be visualized in the CT plan for ideal implant placement during planning. We will contact you to plan the case on line within 3 days of receiving the DICOM and models.

Flapless or flapped surgery

Immediate extraction and implant placement

All implant systems (ie Zimmer, Straumann, 3i, Astra etc)

Surgical guides offer a high degree of accuracy, acceptance may increase due to the lower cost, turn-around is much quicker, and significantly less coordination between the restorative dentist, surgeon, laboratory and patient.

Gallileos CT Scan Solution - Beta

Sirona has developed a device that will make Galileos DICOM available for 3rd party softwares. During the past 5 years we've worked with Galileos owners and Sirona support trying to understand the settings needed to scan an appliance needed to plan surgical guides. The scans are visible on the Galileos monitor in the office, however when it is imported into viewers and other softwares, the images are washed out. Sirona is now testing a device that seems to work. It is a led cylinder that surrounds the appliance, and renders a clear image. We are testing the images that have been produced and will post the results.

No Scan Appliance Technique - Promising Results

We've been hard at work with a few clients testing our no scan appliance technique to fabricating surgical guides. We've had only success so far and will role out our results in the coming weeks. If effective, the process of creating guides will be streamlined to less appointments, less cost, less coordination, and even a higher degree of accuracy.

New Scan Procedure for Immediate Extraction & Placment Guides

1. Separate the primary scan appliance from any removable section (s) attached to the appliance. The separate sections will represent the areas involving the immediate extraction of teeth at the time of implant placement.

2. Seat the primary scan appliance containing the gutta percha fiducials in the patient’s mouth, without the separate sections. Seat the radio translucent bite registration material provided with the case over the appliance and guide the patient into the bite material. In absence of the bite material two cotton rolls can be used.

3. Request that the patient stay motionless, and follow the manufactures instructions to capture the CBCT images.

4. Remove the appliance from the patient’s mouth and attach the removable scan appliance section(s) back to the primary scan appliance. Glue the sections in place with a small amount of adhesive glue (included).

5. After the glue sections have completely dried place the appliance back in the CBCT scanner. The appliance should not be placed directly on the chin rest it should be separated from the chin rest with a Styrofoam or foam block, such as the padding inside a ROE shipping box, figure 3. The appliance should be oriented within the table is the same position as the first scan.

6. Save the data from the two scans into two different folders, Patient & Template, save datasets into one folder labeled with the patients name and upload to www.dentalimplantplanning.com, figure 4. Follow the instructions provided when the “Upload” button is pressed on the left side of the website.

Using a surgical guide to locate and uncover implants AND as a custom tray

The surgical guide used to place the implants is not a throw-away. When the implants are incovered in a few months, place the surgical guide in the mouth, use a marker, place a dot on the implant site and uncover. When the implants are ready for the final impression, use this same guide to transfer the case to the laboratory - an excellent custom tray.
